But why is direct fiat conversion needed? Just do whatever you like with XMR, then if you want to cash out convert to something like ETH or BTC or BCH and cash out?

Could do xmr to btc etc with p2p, but the cashing out part would be hard. How so they pay you for the btc Deposit into a bank will require info Cash on the street? Needs laundered

That's hard since transacting fiat mostly meens involving banks. Unless you do it P2P. And involving banks means you are bound by law to know whom you send bigger amounts of money
