Funny enough the transaction doesn't appear in the Metamask 'activity' tab. Does anyone know something about MM being hacked or why the transaction isn't showing?

Metamask is a hot wallet, which means it's online any time your system is online. If the system where you have Metamask is infected with malware, it's trivial for a hacker to gain access to your Metamask password using a key logger or clipboard monitor. With the password they can export your recovery seed phrase from Metamask, just like if they were sitting at your computer. It's hard to know what actually happened without knowing the details. Any computer can get compromised by clicking on a scam link or visiting a malicious website.
