back in may (assuming it was for a super short amount of time)

Does anyone know why/how that happened?

Oh my bad. Deleted my answer. Ousd got hacked 1 time in Nov 2020 only. If what you said was right, I think it is because someone mistakenly bought on exchanges at that price.

It was confirmed to me by other admins that, there is no OUSD depegging insurance (only smart contract insurance) Has the team said what they will do, if OUSD depeggs and doesn't recover

If OUSD is not hacked and it is depegs, it should be recovered right away. The is only 1 case that could happen which is the underlying stablecoins got hacked.
