1. Sharing your 12 words with unauthorized parties.
2. Storing your 12 words in an unsafe location.
3. Your device being compromised or corrupted.
4. Physical access to your device by an unauthorized person.
5. Using a DApp that had access to your private key and you gave permission to do so, which could have led to unauthorized transfers.
6. Entering your recovery phrase on a fake/clone app.
7. Interacting with a scam token built on a scam Smart Contract.
8. Purchasing a scam multi-signature wallet.

⚠️ Kindly note that confirmed transactions on the blockchain cannot be reversed, making it challenging to recover any lost assets.

What can you do?
If you find yourself in such a situation, the best course of action is to create a new wallet, securely store your new recovery phrase in multiple locations, and abandon the compromised wallet. Depending of the scenario, you could try to revoke access to any smart contracts by using diverse tools such as this.

What can we do?
Unfortunately, there is nothing that we can do once a transaction has been confirmed on the blockchain.

I guess 5 or 7. Please review your wallet transactions, specifically those “approve” transactions. Try to revoke approvals. Better be safe.
