Look above. Hex has 8 decimals. Not 18

Old buggy software like Metamask requires you to manually enter the number of decimals (blockchain databases don't use decimals, only integrals, and the decimals are simulated via "number of decimals").
Tbh, thats the simplest thing to fetch from the blockchain...
I also noticed the address on the ledger screen wasn't my actual address hence the small "100 hex" test but eventually went to my actual address... strange

That is suspicious. Do not sign if the receiver's address on Ledger is not the intended one. You may have a trojan that swaps copypasted addresses.
contact interactions show the contract address on the Ledger screen
It was suspicious but ended up in right address... also the explorer doesn't show it

I think it is but I forgot it now... it was a lot longer than an eth address

On explorer it says ive been transferring out to the Hex contract so no idea. I dont think the ledger is compromised kinda impossible to be honest maybe just a bug

Most ERC / PRC tokens does that, transfers are contract functions so it does go thru the contract
Wallet, Ledger and explorer will show you the contract address of the token. You likely got confused with that being the receiver's address.
