I got "document scan failed", already change the lighting, no glare, focus on document, still got the same error :(

Try also placing it on a dark, even-toned background, if you aren't doing that already

The only other thing I can think of is to check the framing of it, and make sure that it's square and regular in the camera. If you're taking the image at an angle, it might be distorting the result too much.
