But the main reason why I think that people don't upgrade is that it's just not a huge issue. Don't get me wrong, RIR's did run out of "virgin" address space. But there is still a secondary market for address space, particularly in the form of /24 ranges. These cost anywhere between €70-100 monthly. Registration of your ASN meanwhile is best executed through a partnership with an existing member of the RIR you're trying to apply to. Otherwise, prepare to spend thousands yearly on just that. FWIW, RIPE has documentation on all of this.
